TEMPOROMANDIBULAR JOINT (TMJ) TREATMENT

Temporomandibular Joint (TMJ) is a dynamic, living tissue that connects the lower jaw with the skull. Temporomandibular joint dysfunction is a pain and dysfunction syndrome involving the masticatory muscles. Temporomandibular joint dysfunctions have affected a widespread part of the society today. It is caused by the dysfunction of the compatibility between the articular surface and the disc.

Symptoms of Temporomandibular Joint Dysfunction are:

• Pain when opening mouth
• Popping sound and clicking sensation occurring when opening and closing the mouth
• Limited mouth opening (trismus) or locking
• Pain or stiffness in the neck
• Headache (not to be misinterpreted as migraine pain)
• Severe pain felt while chewing and on the face
• Pain when closing the teeth on top of each other
• Quick feeling of tiredness in the jaws while chewing
• Difficulty and pain in opening the jaw, usually when getting up in the morning
• Difficulty in yawning
• Earaches and tingling in the ear, rumbling-like sounds, sensation of congestion and pressure in ears
• Sudden changes in overlapping the upper and lower teeth
• Facial asymmetry, which may be due to hyperplasia of facial muscles
• Deviation (slipping) of the jaw during mouth opening

Causes of Temporomandibular Joint Dysfunctions

• Direct trauma to the jaw or temporomandibular joint area (as a result of accident or blow)
• Unilateral chewing habit
• Missing teeth
• Bruxism (Teeth Clenching and Grinding)
• Stress, anxiety etc.
• Non-physiological tooth occlusion
• Developmental defects of the temporomandibular joint (hypoplasia, etc.)
• Degenerative joint disorders, osteoarthritis, arthrosis
• Autoimmune diseases, rheumatoid arthritis, lupus
• Orthodontic disorders
• Unknown factors

If temporomandibular joint dysfunction cannot be treated with conservative or minimally invasive methods, they should be treated with surgical methods. Tumoural or cystic lesions in the mandible can involve the TMJ and cause extensive destruction of the bone. They require extensive resection after reconstruction. The patient's rib grafts are used for reconstruction together with titanium plates. Jaw reconstruction surgery is a very difficult surgical procedure. Only highly experienced surgeons can perform TMJ reconstruction surgery.

Tumoral or cystic lesions in the mandible can involve the TMJ and cause extensive destruction of the bone. Sometimes it requires extensive resection. Sometimes grafts taken from the patient's rib are used for reconstruction with titanium plates. Jaw reconstruction surgery is a very difficult operation. Only highly experienced surgeons perform TMJ reconstruction surgery. This is a variety of joint replacement surgery.

TMJ ANKYLOSIS SURGERY

TMJ ankylosis, which is one of the causes of inability to fully open the mouth, may be bony or fibrous ankylosis. It can also be unilateral or bilateral. Unilateral ankylosis causes the jaw to deviate towards the ankylosing side. Bilateral ankylosis causes severe limitation of mouth opening, and both speech and eating of the patient are affected. Early mobilization of the joint and exercises in the post-treatment period help the joint function and prevent reankylosis.
